Comparable mortality but higher revision rate after uncemented compared with cemented total hip arthroplasties in patients 80 years and older: report of 43,053 cases of the Dutch Arthroplasty Register

In octogenarians, cemented THA fixation seems to be the safest alternative, nonetheless, there are concerns related to the use of cement in the elderly. This observational register-based study from the Netherlands investigates the risks of death and revision in patients with osteoarthritis aged 80 years and older. Cemented and uncemented stem fixation are compared. The authors find that uncemented stem fixation confers a higher risk of revision than cemented fixation, mainly due to a higher incidence of periprosthetic fractures in this group. Mortality is however similar in both fixation groups, so, sticking to cemented THA in elderly patients seems a reasonable conclusion.

Increased mortality after intramedullary nailing of trochanteric fractures: a comparison of sliding hip screws with nails in 19,935 patients

In a study from the Swedish Fracture Register on 19,935 patients aged 60 years and older with an AO 31-A1 or -A2 trochanteric fracture, the use of intramedullary nails was associated with a slightly increased adjusted risk of death within 30 days compared with sliding hip screws, with no difference at any other time point. This early mortality risk was more increased in association with long nails than short nails, which supports the possibility of fat embolism during introduction of the nails. As there is little scientific evidence to support the use of intramedullary nails in these fracture types, the current trend for using intramedullary nails rather than sliding hip screws may not be justified.

A matched comparison of the patient-reported outcome measures of cemented and cementless total knee replacements, based on the National Joint Registry of England, Wales, Northern Ireland, and Isle of Man and England’s National PROM collection programme

Cemented TKR was introduced 50 years ago. Since then we have spent a lot of time trying to improve the outcome by cementless alternatives and a number of hybrid experiments, including isolated cementation of the tibial stem and patchy cementation under the tibial plateau. In this retrospective observational study the authors instead of revision rates compare the functional and quality of life outcomes for 14 404 (!) matched cemented and cementless TKRs. They found only small, not clinically relevant, differences in PROMs. An important observation however was that the small advantage of cemented over cementless fixation tended to be larger in the < 55 years age group, contradicting the common belief that  cementless fixation is to prefer in younger patients.  That over 95% of TKRs in their National Registries are cemented with lower  revision rates than that for cementless seems to indicate that we, with exception for improvements of implant design and materials, are back to the start of TKR when it comes to fixation.
