Surgery rates for lumbar spinal stenosis in Denmark between 2002 and 2018: a registry-based study of 43,454 patients
Rikke K Jensen, Berit Schiøttz-Christensen, Christian Volmar Skovsgaard, Mathias Thorvaldsen, Rune Mygind Mieritz, Andreas K Andresen, Henrik Wulff Christensen, Jan Hartvigsen
Acta Orthop 2022; 93: 488-494.
Registers are increasingly used, both by authorities and professions, to evaluate different aspects of health care, including processes, rates of surgery for different diagnoses, outcome and health economy i.e. cost effectiveness. Jensen et al. have conducted a register study investigating the development in surgery rates for lumbar spinal stenosis (LSS) between 2002-2018 in Denmark. Both the prevalence of LSS diagnoses and LSS surgery rates more than doubled in Denmark between 2002 and 2018. However, the proportion of patients diagnosed with LSS who received surgery remained stable. Decompression surgery with fusion increased at a higher rate than decompression without fusion, although recent evidence suggests no advantage of decompression plus fusion over decompression alone.

| | Large hospital variation in the risk of dislocation after primary total hip arthroplasty for primary osteoarthritis: 31,105 patients in 59 hospitals from the Danish Hip Arthroplasty Register
Lars L Hermansen, Bjarke Viberg, Soeren Overgaard
Acta Orthop 2022; 93: 503-508.
Hermansen et al. have published several papers in our journal on different aspects of postoperative dislocation after total hip arthroplasty. In the current study they compared the cumulative incidences of dislocation between regions and hospitals in Denmark. By crossmatching between 3 different registers, dislocations treated by revision but also with closed reduction could be included. They found substantial variation between the 59 hospitals, with 2-year dislocation incidences ranging from 0% to 12%. The number of procedures might explain some of the variation, but differences in case mix, surgical technique and positioning of the components, experience and attention from the surgical team, and administrative factors may also play a role. Currently, in most arthroplasty registers the registration of dislocation is limited to cases with surgical exchange of 1 or more components, but register inclusion of dislocations treated by closed reduction should be encouraged, eliminating the need for complicated linking of different databases.
